AFFIDAVIT FOR CHILDS PASSPORT TO BE ISSUED IN INDIA

WE DO NOT HAVE ANY AGENT FOR MISCELLANEOUS SERVICES. APPLICANTS CAN SUBMIT THEIR MISCELLANEOUS APPLICATION IN PERSON AT THE COUNTER IN THE CONSULATE GENERAL OF INDIA, CONSULAR WING, 3 EAST, 64^TH STREET, NEW YORK, NY 10065  BETWEEN 0915AM AND 1130AM ON ALL WORKING DAYS (Monday to Friday, EXCEPT CONSULATE HOLIDAYS)  OR SEND PAPERS BY POSTAL MAIL TO CONSULATE GENERAL OF INDIA, CONSULAR WING, 3 EAST, 64^TH STREET, NEW YORK, NY 10065.

There is no parking space available and the nearest metro station is 59 Street, Fifth Avenue which is five Blocks away.

Please carry originals of all the supporting documents which you have enclosed with the application. The originals have to be shown to the consular officer. If applying by mail, Photocopy of all the supporting documents should be notarized.

Please apply only to Jurisdiction of Consulate General of India, New York (Connecticut, Maine, Massachusetts, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island and Vermont, as well as Puerto Rico and the Virgin Islands).

• There is no need to take an appointment. Applicants could visit the Consulate on any working day between 0915AM AND 1130AM (Monday to Friday, EXCEPT CONSULATE HOLIDAYS). The service will be on first come first serve basis.
• The Consulate reserves the right to reject attestation of documents the contents of which are objectionable or contrary to the Rules.
• The Consulate does not attest any document issued by authorities in India. If any document issued by authorities in India (such as birth and death certificates, affidavits, diplomas, marriage certificate and educational certificates issued by authorities in India ) are required to be presented to authorities in USA, these should be apostilled in India. Each applicant for Miscellaneous Services has to give Indian Community Welfare Fee (ICWF) of US $3.00 w.e.f. October 01, 2012.   

Applying In Person:

Supporting Documents: Carry originals of all the supporting documents to be presented to the Consular Officer (photocopies of which are to be attached with the application). In case you are not able to present the original documents at the counter bring notarized copy of documents being attached to the application. Ensure that notarized copy is legible, otherwise Consulate may ask to see the original. However, the passport and the affidavit (which is to be attested) should be presented in original.

Fees: A non-refundable fee of US$ 10 (ten) per attestation. If you want to receive the attested documents back through mail, either send a trackable pre-paid return mailing envelope (USPS 'Express Mail') or enclose additional money order/cashier’s check for US$ 22 (twenty two) payable to Consulate General of India – New York.
Fees should be paid through Money order or Cashiers’ cheque drawn in favor of the ‘Consulate General of India, New York’. Personal cheques, debit and credit cards or other banking instruments are not accepted.

Supporting Documents: Photocopy of all supporting documents should be notarized and attached to the application. Do not send any original documents except the PASSPORT and the affidavit to be attested.

How to Apply:
• Miscellaneous Services Form - Download the Miscellaneous Application form and complete the application form.
• The affidavit (pdf file) in the prescribed proforma has to be submitted in duplicate along with the passport of the parent staying abroad
• Attach the following documents with the application form.
• Current passport in original and photocopy of the first five pages and last two pages of the current passport.
• Proof of your US Visa Status:(copy of any one of the following)
o Photocopy of the page containing visa on passport (H1B, H4 etc) and the copy of I-94
o Clear Photocopy of Green Card
o Employment Authorization Document (Work Permit)
o I-797, I-140 or I-20 (If approval copy of these notices are pending, also attach a handwritten note detailing the efforts being taken to regularize status)
• Proof of current US residence address: (copy of any one of the following)
o U.S Driving license.
o Gas, Electricity, Water or landline telephone bill displaying applicant’s address
o House Lease Agreement
o State Identification Card
o Note: Bank/credit card/mobile phone statements are not accepted as residence proof
• The Affidavit form needs to be notarized - send original and a copy.
• In case the Consulate receives applications with incomplete documentation, the same would be sent back unprocessed in the return envelope sent by the applicant.
